Who can apply for a new or replacement birth certificate?

Self-applicants must be over 18 years old in order to apply for a birth certificate. If you request a birth certificate for your child, you need to select “Mother or Father” during the registration phase.

If you are not able to make the request yourself, you can assign a person/entity you trust to be your legally authorized representative. The legally authorized representative will have the authorization to obtain your record on your behalf. The document must be signed by both parties in front of a notary.
