The Health Department of requires applicants, once they have completed and signed the application form, to prove their identity by including a photocopy of their ID. Acceptable document of identification are:

If you can provide documentation of legal interest, or if you are holding a Power of Attorney from the person on record, you are eligible to request the vital record for that person.
How long will birth certificate delivery take?
It will take about 8 to 10 business weeks to get your certificate. The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.
