The Health Department of requires applicants, once they have completed and signed the application form, to prove their identity by including a photocopy of their ID. Acceptable document of identification are:Provide one (1) of the following identification documents:
- Driver's license
- State Id
- Us/Foreign Passport
- Military Identification Card
- Permanent Resident Card (green card)
Who may apply for birth or death certificate?
Please see the following list of person authorized to order a vital record:
- Person on records
- Legal guardian
- Grand child
- Grand parent
- Legal representative
If you can provide documentation of legal interest, or if you are holding a Power of Attorney from the person on record, you are eligible to request the vital record for that person.
How long will birth certificate delivery take?
It will take about 5 to 10 business days to get your certificate. The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.
How long will death certificate delivery take?
It will take about 7 to 15 business days to get your certificate. The state needs ample time to process the number of inquiries they receive. Delays may vary depending on the volume being processed.
