An amendment is essential when there is a need to change the information on a New Jersey marriage record. Different forms are required depending on the type of amendment needed. While the original marriage record cannot be altered, supplemental changes can be added.
You can request changes to personal information, such as minor spelling corrections in a name, date, place of birth, or residence, provided you have the necessary documentary proof.
When submitting an Application to Amend a New Jersey Vital Record form, a copy of documentary proof must be included. The documentation must display the individual's full name and date of birth. Acceptable forms of evidence include a birth certificate, marriage certificate, divorce record, school admission records, court order, certificate of naturalization, petition for name change, baptismal record, hospital or medical record, or child immunization record.
A driver's license, a social security card, or a decorative birth certificate issued by a hospital cannot be used as proof.
The requirements for amending a marriage certificate will require the following:

- Appropriate fee(s): the search fee with a copy of the amended record is $25.00; additional copies ordered simultaneously are $2.00 each. Make check or money order payable to "Treasurer, State of NJ.”
