Application Process for Correcting Certificates
Where to Apply:
To correct or complete a certificate, apply to the city or town clerk where the event occurred. The fee is $10.00; if you need a certified copy, there's an additional $15.00 per copy. Make checks payable to the respective City/Town Clerk.
How to Apply:
Use the "Application For Correcting or Completing A Certificate of Birth, Marriage or Death (FORM VSCr)" from the county clerk's office. Complete, triplicate, and sign it with notarization. For minor changes within six months, a different form (VSX) will apply, selected by the town clerk.
Evidence Required:
Along with the notarized form VSCr, submit two pieces of documentary evidence dated close to the event. Acceptable documents include baptismal records, hospital records, early school records, and affidavits of personal knowledge (notarized). Photocopies of documents can be used if certified by the original holder. These documents will be returned after the certificate is amended.
Correcting Names:
If there's a discrepancy in a name, it can be corrected by presenting two early pieces of documentary evidence showing the correct name.
